Description (Service) Consists of ten family physicians and a team of allied healthcare professionals including; 
* Nurse Practitioner * Diabetic Educator * Health Promoter * Dietician * Social Worker * Chiropodist 
* Pharmacist * Physician Assistant 
 
 
Services Include; 
* Dietary Counselling - Are you coming to visit our Dietitian? Please print and fill out our food diary four days prior to you appointment. Food Diary 
* Chronic Disease Management – Education, prevention & follow up care. 
* Mental Health Services – Social worker, children, teenagers & seniors. 
* Women’s Health – Early Detection and Education 
* Health Promotion Prevention – Education, Nutrition & Exercise 
* Neuro-Muscular Diseases – Parkinson’s, MS, Alzheimer’s

Boundaries Serving patients from within the Townships of Brooke-Alvinston, Plympton-Wyoming, Enniskillen, Warwick, St. Clair and the town of Petrolia.
