Description and Service Details
Description (Service) Provides an innovative opportunity to young adults with a developmental disability who are leaving the educational system. Also provides the opportunity to continue to learn, develop skills, and enhance personal growth. offers Person-Directed Planning, goal setting, time-limited services up to a maximum of 5 years and support during transition.
