Description & Services
Description (Service) Provides primary health care services to identified priority populations living within a specified catchment area*. Provides a wide range of health promotion and disease prevention programs to all community members. Specialty Teams: Diabetes Education Program, COPD, Falls Prevention, and Cardiac Rehab available to all community members.
Hours Mon and Fri 8 am-4:30 pm * Tue-Thu 8 am-8.00 pm
Boundaries Mandaumin Road to the east, Lake Huron to the north, St. Clair River to the west, and the Lambton County boundary to the south
Eligibility For Primary Care: must be low income, a senior, a youth at risk, and/or have a moderate mental health problem; live within the catchment area; and not currently have a primary care provider
