Description (Service) Transitional Case Manager facilitates and supports the transition of inpatients of forensic units through creating and supporting community placements

Forensic psychiatric services include assessment, treatment and rehabilitation of people with mental illness who have been in significant contact with the law * support with transition to the community, counselling, follow-up, crisis intervention, and linking to outpatient community mental health agencies * works in collaboration with the Forensic Outreach Team of Southwest Centre for Forensic Mental Health Care
