Description & Services
Description (Service) This program provides services to eligible seniors, who due to lack of resources or appropriate supports, require assistance in returning home after a hospital stay. The goal is to support the independence of our clients by providing transitional support services from hospital to home. Services may include transportation, meals, home management (essential laundry, tidy up, grocery shopping) and companionship. * Maximum of 7 hours of scheduled service to be completed within three weeks after discharge from hospital.
Hours Mon-Fri 8:30 am-4:30 pm
Eligibility Ages: 60 year(s) and up 
- Independently mobile 
- Requiring assistance to return and settle at home 
- Able to be left alone when service ends 
- Not requiring assistance with personal care or medical care from Homeward Bound
