Description (Service) Dedicated to enhancing the health, quality of life and independence of individuals in our communities by offering a single point of access to home and community services. Our services include: case management, specialized nursing programs, nursing clinics, personal support services, occupational therapy, physiotherapy, social work, speech-language pathology, information and referral to community health and social services, services for children and youth, palliative care consultation, telehomecare and much more.
Hours Mon-Fri 8:30 am-4:30 pm
Eligibility Assessment are done to determine service needs and eligibility.
Application Anyone can make a referral to the Home and Community Care Program – you can refer yourself, or have a family member, friend, caregiver, physician, or other health care professional make a referral
